Let us know the date and approximent time that you would like to make your cat's next appointment. We will then check for schedule availablility and either email you back with a confirmation of your original appointment request, email you with alternate times if the original time is already booked, or call you to discuss your upcoming appointment.

Please be aware that this is a request for appointment only. Your appointment is not on our schedule until you receive a confirmation email from us with the date and time of your cat's appointment.

If your cat has an illness or otherwise needs to be seen immediately do not use this form. Please call the clinic at 336-545-3390 and we will schedule you in as soon as possible
